PAYMENT & FEES • Accepted forms of payment: Visa, MasterCard, Discover, American Express, Check, Cash or Money Orders. For security purposes, identification will be asked to be provided for credit cards and checks. • A $3 administrative fee applies to all transactions. • A $15 fee applies to all other non-residents (anyone living outside Fontana and not in the adjacent unincorporated county area). • Some classes may require additional fees for class materials/supplies paid to the instructor. It is recommended the purchase of required supplies or class attire be done at the first class meeting. • All checks returned for insufficient funds will be charged a $38 administrative fee, in addition to the class fee. The total fee will need to be paid in cash or money order at Fontana City Hall. REFUND/CREDIT/TRANSFER POLICY • If you are unable to attend or are not satisfied with a class, you may transfer to another class (if space is available), request credit for future Community Services Department program or request a refund. Requests for transfers and refunds must be made prior to the second class meeting. • Please allow 4-6 weeks processing for all refunds. • Approved refunds are subject to an administrative fee of $10 per participant, per activity, per membership, per facility reservation. • Material fees paid to the instructor are non-refundable. CLASS INFORMATION • All classes/activities are subject to change or cancellation. • All fees, times and dates of all programs are subject to change. • Children are required to be supervised at all centers and programs. • Child care is not provided prior to, during or after classes. • Clubs may be led by City volunteers. • All instructors are contractors or City of Fontana employees. Instructors may use independent assistants or sub instructors to teach classes. • Successful operation and delivery of recreation programs and classes is dependent upon appropriate staffing levels. • Instructors have the option of allowing parents to remain in the class. MINIMUM & MAXIMUM ENROLLMENT Classes will be canceled or combined if they do not have the minimum amount of registered participants. Classes with full capacity registration will begin placing interested participants on a waiting list. When and if openings occur, participants will be contacted and registration will take place on a first come, first serve basis. Release of Medical and Liability & Assumption of Risk Form and Registration Fee are due at time of registration. PHOTOGRAPHY DISCLOSURE All participants involved in any City programs or special events are subject to being photographed. Such photographs may be used by the City of Fontana for marketing and promotional purposes. Official City photographers are required to display an official City of Fontana ID badge at all times.
